My name is Dr. Holly Chalk, and I am an Assistant Professor of Psychology at McDaniel College. We are conducting a study on behalf of the E. Diane Champé Institute. The survey results will be analyzed to identify and potentially substantiate needs of adult survivors of child abuse and neglect for specific services.

Voluntary Nature of the Study:  Participation in this study is completely voluntary. You can choose to end participation at any time, without penalty. You may also skip questions that you are not comfortable answering.

Procedures:  If you agree to participate in this evaluation, you will complete a brief survey either on line or in print form. This survey is expected to take 10-15 minutes. You are asked to complete this survey only one time.

Risk and Benefits:  The benefit of this study includes the opportunity to contribute to a knowledge gathering effort that could result in future services for adult survivors. Risks of participation are expected to be minimal, but some questions are personal and have the potential to make you feel uncomfortable (e.g., questions about mood or safety).  You have the right to skip any questions that make you feel uncomfortable and the right to withdraw from the study at any time.

Confidentiality: The information that you provide will be kept confidential. You do not need to provide any identifying information. All questionnaires will be sent directly to McDaniel College.  My research assistants and myself are the only ones who will see the answers to the questionnaire, and no identifying information will be tied to your responses.
