In order to help us to provide you with the most relevant and useful resources in an efficient manner, please provide us with as much of the following information as possible:

- your medical insurance provider
- mental health diagnosis for which you are seeking help
- the type of treatment and/or support you are looking for, e.g. therapist, psychiatrist, support group, treatment center
